0345 (3c, d) During the course of the complaint assessment, Licensor Champney obtained enough information available to determine children were allowed leave the Licensed space unsupervised to use the restroom. 0345 (5c, i) Staff was not providing supervision when children come in close contact with the household pets (pets were not safely secured in the pathway to the restroom where children travel).
